FALL SPORTS SIGN UPS ARE NOW IN THE NURSES OFFICE
please be aware of the school's concussion policy.
ATTENTION:
NO ATHLETE WILL BE ALLOWED TO TRY OUT WITHOUT A CURRENT PHYSICAL AND ALL
PAPERWORK ON FILE! This includes:
- athletic health history (to update any changes in health)
- medical release form (every season)
- contract (if not done already)
- physical on file (completed within last year)
NYS Education Law requires all students in 10th grade to have a current
physcial on file, including BMI (body mass index percentile).
Forms available in the Nurses Office at this time, continue to monitor the
website for forms on-line.
All 10th grade students also will be having vision/hearing screenings in the
nurses office throughout the year. Any other students will be screened based
on request either by the student/teacher/parent.
All 9th grade students will be screened for scoliosis throughout the year as
well.
