Participating in the School-to-Work Program or any job shadow or internship requires alot of paperwork.
There are certain forms that must be on file according to the New York State Education Department, as
well as, the Pulaski and Sandy Creek School Districts. To access the necessary forms, simply click on
the link and download the form. Fill it out completely (incomplete forms will not be honored) and drop
off or mail to Mrs. Crast.
All forms must be filled out and include required signatures before a student will be allowed to begin
their job shadow/internship.
