       ***ATTENTION PARENTS***
Parent Teacher Conferences are upon us!

October 20, 2009

Dear Parent/Guardian:

You are a vital partner in the education of your child, and parent-teacher
conferences play an important role in helping us plan and implement programs
that meet the expectations you have for your child’s education. 

With this in mind, Van Wyck will be hosting sixth grade parent-teacher
conferences, on Thursday and Friday, November 19th and 20th as well as Monday,
November 30th.  Each day, the conferences will begin at 11 a.m. and will end
at 3:00 p.m. In addition to the afternoon conferences, we will have one
evening session, on Thursday, November 19th, starting at 6 p.m., and ending at
8 p.m. Please be advised that all sixth grade students will be dismissed from
school at 10:30 a.m. on each of the three report card conference dates.

In the near future, teams will be sending out invitations for parent-teacher
conferences. If you receive an invitation to attend a conference, please call
the following secretary, according to your child’s team, during normal
business hours:

	Teams 6A, 6B and 6C 	Mrs. Cathy Keller	 	227-1700 x 108

	Teams 6D, 6E and 6F	Mrs. Kathy Romanovsky 	        227-1700 x 103

Each conference usually lasts 15 to 20 minutes. We must adhere strictly to our
schedule so as to avoid delaying others. Students and/or other children should
not attend the conferences. 
 
If you were not invited to attend a conference, but would like an opportunity
to discuss your child’s progress with his/her team, please call the designated
secretary to inquire about available conference times. If you are unable to
schedule a parent teacher conference during this time, please call Mrs.
Romanovsky to schedule a team meeting.

      *** TdAP Information***

  The following is an informational letter regarding the Tdap immunization
incoming 6th grades students are required to have, per NYS Law. (Exemptions
are also explained in the letter). 

Dear Parent/Guardian,

The New York State Department of Health has established a recent
change in the immunization requirements for school entry related to
diphtheria, tetanus and pertussis vaccine that has taken effect since
September 1, 2007.

Students born on or after January 1, 1994 and who are entering/
repeating grade 6 or a comparable age-level special education school
or program on or after September 1, 2007 and who are 11 years of age
or older must receive the immunization containing tetanus toxoids,
diphtheria, and acellular pertussis.  The acronym for this vaccine is
Tdap.

There are two exemptions from this requirement:
1. A medical exemption consisting of a written statement from a
physician licensed to practice medicine in the State of New York
stating that there is a valid medical contradiction to the student
receiving the booster vaccine.
2. A statement of religious exemption, written by the parent/guardian,
stating that they hold genuine and sincere religious beliefs which
prohibit the immunization of their child.  A “Request for Religious
Exemption to Immunization” form must be completed, notarized and
submitted to the Office of Student Services.  You will be notified in
writing of the outcome of this request.

Please forward the required information to the school Health Office by
September 9, 2009.

Team D - Ms. Evangelisti, Ms. Sheahan and Ms. Swanson
Team E - Mr. Barysh, Ms. Humphreys, Ms. Garvey, Ms. Pinto (formerly Ms.
Donohue, congratulations!)
