Health Survey
You are to create a survey, similar to those found in magazines, newspapers, etc., that rates someone’s heath.
Requirements:
· The questions should reflect the age group this survey is aimed at.
· Directions should be specific.
· 25 or more questions that do not repeat.
· Questions must cover Mental, Physical and Social Health.
· There must be a grading system and a ratings system. (How many points for what answers and what does the resulting score mean)
For example:
Survey: Answer True or False to these questions:
1) Do you smoke cigarettes?
2) Are you exposed to cigarette smoke at least 3-5 days per week?
3) Do you use sun screen when going to the beach?
4) Have you ever shared drinks with another person?
5) Does your job have you work outside?
Grading and Rating:
For each True answer give your self 2 points. “False” answers get no points.
· 10 points - 7 points: You are at a high risk of cancer exposure.
· 6 points – 4 points: You are at a medium risk of cancer exposure.
· Less than 3 points: You have a low risk of cancer exposure.
