6th Grade

IMMUNIZATIONS NEEDED FOR ENTRY INTO 6th GRADE:

All entering 6th grade students born BEFORE September 2 will be required to RECEIVE a booster dose of Tdap [Tetanus,Diptheria,and acellular Pertussis (Whooping cough)] when they turn 11 years old. This immunization is also known as Adacel. Those 6th grade students born AFTER September 2 will be required to provide proof of immunization of Tdap within two weeks of their birthday in order to remain in school or they may receive a Boostrix immunization when they are 10 years old. In addition, all incoming 6th graders must also show proof of immunization and/or immunity to Varivax [Chicken Pox]. Proof of immunizations must have a physician's signature and can be mailed to the Health Office .

Students WILL NOT be permitted to attend classes without proof of having the  REQUIRED Tdap immunization AND immunity/ vaccination to chicken pox.

SCREENINGS:

All 6th grade students will be given a scoliosis screening in school in the Winter or Spring during their gym period. If a scoliosis screening is completed by your Physician during the current school year after June 2009, please send a copy to the Nurse's Office by October 15th and your child will not have this screening at your request.

Vision and hearing screenings will be done periodically if a problem is detected.
