Welcome Letter from Susan Berry, Principal

The registration process for 2008/09 for kindergarten students is threefold:

1. Completing and returning the registration forms

2. Pre-Kindergarten screening (DIAL-3); Vision/Hearing Screening

3. Kindergarten Orientation/Bus Ride

Registration:

Please complete the registration forms that were mailed to you
and return them to Mohansic School by Wednesday, March 12, 2008.

Below are electronic copies of those orginal forms. You will need Adobe Acrobate Reader to open them.

1. District Emergency and Emergency Health Information cards.
2. The Yorktown Central School District Registration Form
3. Preschool Release of Records form.
4. Home Language Questionnaire-HLQ
5. Parent Questionnaire for Kindergarten Intake-required
   Parent Questionanaire for Kindergarten Intake-optional (additional child information)
6. Application for Transportation to and from day care location (if applicable). Must be submitted no later than April 1.
7. A copy of your child's birth certificate to be kept on file at the school.
   An original birth certificate (with official seal) must be presented for review at the time of your screening appointment.

Schedule your child's DIAL-3 Screening

• You must call Mohansic to schedule your child's DIAL-3 Screening and Kindergarten Orientation and Bus Ride appointments
• Students with Last names beginning with letters A through M please call only on March 10th, between the hours of 9:00 AM and 2:30 PM. (234-7235)
• Students with Last names beginning with letters N through Z please call only on March 11th, between the hours of 9:00 AM and 2:30 PM. (243-7235)

DIAL-3 Screening Dates:

Your child wil be screened for kindergarten readiness, by appointment only on the following dates:

Monday, May 5

Tuesday, May 6

Wednesday, May 7

Thursday, May 8

Friday, May 9

Monday, May 12

Tuesday, May 13 (if numbers make this date necessary)

Your appointmenst will last approximatley 1-1/2 hours; with your child having his/her vision and hearing screenin immediatly following the DIAL-3 Screening. Your child will be screened by Mohansic staff in the areas of language, concepts and motor skills. Arlene Klause, our psycologist, will coordinate the screening and share with you the results immediatley after the session. Jucie and cookies will be offered to students, so please notify us if there are any dietary restrictions or allergies.

The Orientation/Bus Ride

The Orientation/Bus Ride will be held on Wednesday, May 28th by appointment. We will have a morning and an afternoon session. Children will visit the kindergarten classrooms and take a short bus ride, while parents meet with the principal and discuss the kindergarten program.
