Pied Piper Application Form


 

 


Welcome! The Pied Pipers is a performing organization that focuses
upon performing for pre-school through lower elementary age range, as
well as for the elderly.

We do have fun in rehearsals and at performances! But, due to some
issues that happened last year, it is important to clarify the
following information. These requirements are not for the purpose of
taking away the "fun" and "excitement" of this performing ensemble,
but to make sure we are all "on the same page" concerning commitment
to this great organization.

Parents, please verify you have read the requirements (found on
another page in my website) for being a Pied Piper by
answering "yes" type of short answers for statements 1 - 5. Numbers
6 and 7 provide a way for you to type in longer responses.

When I receive this application, I will send you a notice of
verification. Thank you for letting me "borrow" your child for the
purpose of serving our community in such a delightful way.

When you complete this application, push the "submit" button and
your form will come directly to my e-mail. Musically, Mrs. Rezach

  • 1) We have read the requirements for being a Pied Piper and agree with
    the requirements.

    Answer:
  • 2) We understand we need to provide transportation for our child to and
    from all performances.

  • 3) We understand our child will need to purchase a uniform and that
    she/he will not be able to perform with the Pied Pipers until he/she
    has a uniform. (An exception will be made if the uniform has been
    ordered and the company is late in getting the uniform to the
    child.)NOTICE: If your child purchased a uniform last year, you do
    not need to purchase another one. We will use the same uniforms.

  • 4) We understand that commitment is of utmost importance since the Pied
    Pipers only meet once a week: thus, rehearsals are mandatory. We
    will make sure our child considers the rehearsals top priority.

  • 5) We understand that our child will be using the computer to learn the
    Pied Piper repertoire. We also understand he/she should check the
    computer on a weekly basis. (We will alert our child when we
    see "news flashes" in our e-mail coming from Mrs. Rezach. These
    news flashes will include important information every Pied Piper
    will need to know.
