Attention: I need a parent signature; therefore, please print
this form once you have filled in the two questions/statements below
and sign the paper. Bring the signed paper to me tomorrow, September
15th so you can ride the bus.

Please verify that you are in agreement that your child may travel
to Roberts Middle School via the school bus beginning on Tuesday,
September 15th and every Tuesday following.

You will be responsible for picking up your child at Roberts each
Tuesday at 5:00 P.M. (We always stop rehearsals on time.)

The bus will leave Bolich at the end of the school day. The time and
bus number will be announced to the students once the bus garage
gives me this information. Thank you, Mrs. Rezach
