Athletic Participation Forms

Please read the information below if looking for forms, as the athletic forms
requirement has changed for the 2012-2013 school year.

Here is a link to the Orange Schools Athletic page, where you can get copies
of the required forms for athletics.  The 2 forms you will need are the
"Participation Requirements/Emergency Procedure" and the "Physical Form". SEE
BELOW FOR AN UPDATE.

UPDATE-  USE THIS LINK FOR THE NEW OHSAA PHYSICAL FORM, EFFECTIVE FOR THE
2012-2013 SCHOOL YEAR.  PLEASE BE SURE THAT THE "DATE OF EXAM"PORTION OF THE
PHYSICAL IS FILLED OUT BEFORE TURNING IT IN.
