Dear Parents and Students,

 

        Welcome to fourth grade!  We are looking forward to a great year and would like to take this opportunity to share some important information with you.  Please take a few moments to read through the items in this folder.  Please pay special attention to our Introduction to Fourth Grade informational packet and share this information with your child.  The complete Fourth Grade Handbook is available online at our website.  If you have any questions or concerns, please feel free to contact us.  

 

The following items should be completed and returned to school with your child no later than Tuesday, September 8.

 

ü     Introduction to Fourth Grade Verification Form

ü     Student Survey

ü     Student Emergency Card Update (Please make sure all sections are complete.)

ü     Student Internet Usage Agreement and Drug Policy

ü     Free and Reduced Lunch Forms if eligible (Delivered U.S. Postal Mail)
