If you are completing your experiment in a lab/and or research center, you must complete the form.

If your project involves bacteria, mold or chemicals, you must complete this form.   If you are unable to work with qualified scientists, please see me during lunch on Monday.  I will be able to secure qualified scientists.

All projects that state designated supervisor

Any projects that involve a student or adult participant, this form must be completed prior to the experimentation.  Do not have your parents or you sign this form.  The signatures are for the parent and or participant.
