I forgot to bring my Personal ID for Act 48. Can I send my forms in later?

No.  Forms must be submitted at the time of the staff development program. 
Since it is a 7-digit number, we suggest that you plug it into your cell 
phone with the name "Act 48" so that it is easily accessible.

No.  This practice is no longer acceptable.

Where do I get Act 48 forms?

Forms for approval of professional development activities are located on the
Act 48 page of this web site.  Forms used at the end of workshops to request
hours and evaluate the session are distributed at the workshops.

I did not find my hours from last year posted. Can I apply for them now?

Ample notice was given in the spring announcing the closing of the "window"
for the previous cycle.  These should have been checked  prior to June 30. 
There are also several possibilities for non-receipt of hours:
        -Activity was not pre-approved
        -Activity was not appropriate for teacher's teaching assignment or
area of certification. (e.g. a kindergarten teacher can not earn hours for a
course/workshop on Biology.)
        -The form may not have been received by us. (We can check on this.)
        -The teacher did not have Level II certification at the time of the
completion of the activity.
        -Forms were incomplete--missing information or documentation required
to validate acceptability of activity.
