Adult Volunteer Forms .... 
 
Please note:  
  • Your signature on the forms must be witnessed by school personnel. Do not sign the forms at home.
  • Appointments are being scheduled for TB tests during the week of September 28 and October 5, from 7:45 a.m. to 3:15 p.m.
  • You may call Carol Confer, Secretary to the Superintendent, at 610.682.5100 for answers to your questions.

Child Abuse Clearance   

 Parents and students, if you would like to request funds from your student credit, please print the Student Credit Form from the "Forms and Documents" link on the left menu. Forms may be submitted to Mrs. Rhodes or Mr. VanVoorhis. You may also pick up a form in the band or choir room.
