Parents / Guardians can register their child for school by stopping in the
Dallas Middle School central office between the hours of
8:00AM – 3:00PM, Monday through Friday.


You will need to bring with you following information:


· A copy of your child’s birth certificate

· A copy of your child’s immunization records

· A copy of your child’s most recent report card and most recent
standardized test scores

· The full name, address and telephone number of your child’s
previous school

· Information regarding any special needs your child may have
(including I.E.P., Service Agreement, etc.)

· If applicable, a copy of any legal documents regarding child
custody agreement etc.
