H1N1 (Swine Flu) Vaccine at GNA

The GNA School District has been selected by the PA Department of Health as a provider for the H1N1 (Swine Flu) vaccine. The district ordered a begining supply of vaccinations from the Pa. Department of Health and the clinic will be held based on the delivery and amount of supplies we recieve. We will start by administering the vaccine to any of our students who are interested AND return a consent completed and signed by the parent or legal guardian. You will be receiving in the mail (High School) or your child will bring home an informational packet with forms that need to be completed and signed if you choose to have your child participate in the vaccination program.
 
If you wish for your child to have your child recieve the H1N1 vaccination, complete and sign all forms and return them to your child's homeroom teacher no later than NOVEMBER 5, 2009. WE ask all parents to take their time and read all of the information BEFORE your sign and return the consent forms. If you have any questions or soncerns please visit our website for additional information or you may call the school to express your concerns and we will try to best answer these questions in a timely manner.
 
If your child has not come home with the papers concerning the H1N1 vaccinations, the informational packet and consent forms can be found on the school webpage.
