ADD TO THE POLICIES, RULES AND REGULATIONS CONTAINED IN THIS HANDBOOK AND TO
MAKE SUCH CHANGES APPLICABLE TO CURRENT AND NEW STUDENTS WHEN THE SITUATION
DICTATES.
RELIGIOUS PRACTICE
Holy Spirit Catholic School expects each child to practice his/her given
religious beliefs. Even though the parish school respects and enrolls
students not of the Catholic religion, Catholic religion classes and
liturgical participation during the school day are required for all students.
In addition all children are to participate in weekly worship in their
respective parishes. Holy Spirit parishioner children are required to use
their weekly youth envelope as one measure of weekly church attendance.
No specific amount is required in the weekly offering envelope.
SACRAMENT PREPARATION
Holy Spirit’s religious education program prepares both parishioner and
non-parishioner Catholic students for the reception of the sacraments of
Reconciliation, First Eucharist and Confirmation. Holy Spirit School also
prepares non-baptized children for Baptism upon request. Preparation and
reception of a Sacrament is a privilege, not a rite of passage connected with
a particular grade or age. In order to participate in the reception of a
sacrament at Holy Spirit, the following is required:
™ Holy Spirit children must be attending weekend Mass, and using the
sacraments he/she has already received, consistently.
™ Catholic students from other parishes must present a letter from their
Pastor indicating that the child is a practicing member of the parish and
that the Pastor gives permission for the child to receive the sacrament at
Holy Spirit.
™ At least one parent must attend the required number of parent sessions or
makeup sessions for each sacrament.
™ All student work required in the sacrament preparation program must be
completed.
EIGHTH GRADE GRADUATION EXCERCISES
Since Holy Spirit School is a Catholic school, the official graduation
ceremony is a religious one, traditionally a Mass. In order for a parish
eighth grader to participate in the graduation Mass, he/she must attend
weekend Mass consistently. The use of the church envelope is one method of
monitoring such attendance, but not the sole method. For those Catholic
students from other parishes, a letter from their respective pastors,
indicating consistent weekend Mass attendance will be required in order to
participate in the religious graduation ceremony.
Closing Exercises
Participation in closing exercises is a privilege, not a right. The School
has
the right to deny any student from participating in closing exercises if, in
view of the School, the student’s conduct or academic or disciplinary record
indicate that the privilege should not be extended.
TUITION
Tuition payments will be collected through Mrs. DiGiovanni at the Holy
Spirit Rectory. You will have the option of paying your tuition in two or
three installments, or in ten monthly installments. A 5% discount will be
given for payment in full if received by June of the prior school year.
Payments will be due on the first of the month, unless otherwise indicated,
with the first payment due July 10 th . If your payment is not received by
the
first of the month, you will receive notification on the next school day, or
by mail if in the summer. Any payment not received by the 15 th of the month
in which it is due will be subject to a late fee. The late fee will be
$15.00. If payments become delinquent by more than 60 days, your child or
children will not be permitted to return to school until payments are made
current. Payment schedule will be as follows:
Two payment plan: Due July 10 th and January 5 th
Three payment plan: Due July 10 th , November 1 and March 1 st
Ten payment plan: Due the first business day of each month starting
July and ending April.
ATTENDANCE`
The school doors open at 7:55 AM. Students in pre-k should be brought
directly to the classroom by the parent/guardian. Students in kindergarten
through grade 8 report directly to the school hall and sit in their assigned
seats. A teacher will be on duty to supervise the students. For the safety of
the children, parents or other adults are not permitted in the school hall
without the permission of the principal. The bell rings at 8:10 AM.
*PLEASE NOTE: NO CARS ARE PERMITTED TO DRIVE INTO THE SCHOOLYARD TO DROP OFF
CHILDREN EXCEPT IN THE AREA OF THE LARGE SCHOOLYARD BEHIND THE RECTORY
WHERE THE BUSES PARK.
Students may not be dropped off in the schoolyard before 7:55AM. The
school is not responsible for students or parents/guardians on the premises
during unsupervised times.
On the following days, students go directly to their classrooms: election
days
and Raffle Night.
LATENESS
Punctuality is important for academic success. Lateness not only interferes
with a student’s learning, but also causes disruption for the teacher and the
class. A record is kept of all lateness and if any child arrives in school
consistently late, the principal will contact the parents in order to correct
the irregularity. Any student who is late is to report to the office for a
late slip and to inform us of the reason for being late. The student
presents
the late slip to his/her homeroom teacher for admission along with a note
from
the parent explaining the reason for the lateness. Repeated lateness will
result in consequences determined by the principal.
Consistent unexcused lateness will be considered parental neglect, which
will be reported to the Philadelphia School District for further
investigation.
Lateness, of course, impacts on a perfect attendance record and constitutes
a serious infraction.
ABSENCE
Each time a student is absent from school, the parent or guardian must
phone school (215-389-0715) before 9:00 AM each day of the absence and
then write a note when the student returns to school to explain the reason
for
the absence. A parent or guardian must provide a satisfactory written
explanation for the student’s absence from school. A doctor’s note is
required after three days of absence. It is the responsibility of the
student
to make up any schoolwork that has been missed during the absence. Any
work not made up could significantly alter the student’s grade.
LUNCH REGULATIONS
The lunch period runs from 12:15 to 1:00 PM. For safety reasons, the only
adults permitted in the school hall or schoolyard are lunch monitors or
kitchen workers.
- Students must sit in their designated seat except when purchasing food or
using the lavatory. Students may not visit other family members in a
different grade without the permission of the principal.
- Students are responsible to dispose of their own trash before leaving the
school hall.
- Students should always be respectful of the lunch monitor, kitchen
workers and one another. If a parent or child is unhappy about an
incident which occurred at lunch, the parent should address this issue
first with the child’s teacher and then the principal if the issue is still
not resolved. At no time may a parent approach the lunch monitor or kitchen
worker.
- Students who go home for lunch and do not pay a lunch fee must go home
everyday except for special events or inclement weather. The student
must have a note stating that he/she is to go home for lunch. The student
must sign out in the school office before leaving school and sign in when
returning to school.
- Continuous disregard of lunch rules will result in the loss of in-school
lunch privileges.
- If a parent wishes to have a child leave school at lunchtime, prior
arrangements must be made through a note sent to the homeroom
teacher and forwarded to the office. The child must have an adult with
him/her when leaving for and returning from lunch.
DISMISSAL
School dismisses at 3:00 PM. Parents of children in Pre-K and
Kindergarten are asked to go directly to the classroom to pick up the
children. Parents of children in grades 1 to 8 may either park in the
schoolyard or on Geary Street or Hartranft Street. In order to ensure the
safety of our school students, no one is permitted to double park on either
of
these streets or to drive into the church side of the schoolyard.
*Please note: Entry is not permitted into the school building before
school in the morning, at lunch recess, or after dismissal or at any time to
pick up forgotten items unless the classroom teacher gives permission.
A child will not be released to a parent/guardian that does not have physical
custody, without the written consent of the custodial parent/guardian. To
determine the custodial parent/guardian, all separated or divorced parents
of children enrolled in the parish school must provide the school with a copy
of the court order or custodial agreement adjudicating that determination of
custody. This Court Order/Custodial Agreement is placed in a confidential
file.
TELEPHONE
The telephone is used to transact business only. No student may be called to
the phone during school hours except in case of emergency. Students are
not permitted to use the phone except in an emergency. Forgotten articles of
any kind, books, homework, gym clothes, etc. are NOT considered
emergencies. Phone calls are at the teacher’s discretion.
Daily Schedule:
Morning session – 8:15 –12:15
Lunch – 12:15-1:00
Afternoon session – 1:00-3:00
DOCTOR APPOINTMENTS
Unless there is a medical emergency, doctor and dentist appointments
should be made outside school hours. If an appointment must be made
during school hours, the student should bring in a note signed by the parent
or guardian stating the reason and the time of the appointment. The note
should be given to the classroom teacher. The parent or guardian must
report to the office to sign the child out of school.
VACATION
If, because of family obligations a student takes a vacation during school
time, the student must present a note from the parent to the principal at
least one week in advance of the vacation. The student is responsible to get
all missed assignments the day he/she returns. The student will have three
days to complete all written work. No teacher is required to assign work
beforethe rest of the class receives the assignment. Teachers will not be
expected to reteach the material a student missed while on vacation. Parents
should help with this material. Unfinished work will result in a zero for
each
incomplete assignment.
EMERGENCY SCHOOL CLOSING
Parents should listen to KYW news radio 1060 AM to determine whether
school will be closed due to inclement weather. Schools located in
Philadelphia have no closing number. Our school will be announced as
“All parochial schools in Philadelphia will be closed.” Please do not call
the rectory or school.
If school is in session during an emergency closing, you will be contacted by
the school phone chain. Thirty minutes prior to the announced closing time,
students will be brought to the school hall for dismissal. Parents are asked
not to come to school before that time as this interrupts school instruction
and makes it confusing for teachers and students alike.
COMPLETION OF SCHOOL WORK
The primary purpose of any Catholic educational instruction is to enable
each student to develop his/her spiritual, academic and social abilities to
their fullest potential. In order for each student to reach this potential,
it is of utmost importance to instill in each student a proper attitude
toward
school and learning.
Parent/Guardians should encourage children to:
- Take pride in work well done, and to strive for excellence in each task the
child is asked to perform.
- Start and complete his/her homework within given time limits.
- Seek help when needed.
- Assume a degree of responsibility for his/her supplies.
HOMEWORK
Homework is a reinforcement of the teaching in the classroom and a
preparation for class. Parents or guardians are asked to help their children
develop good study habits. Ability may affect the amount of time each
student needs to complete homework. Written assignments should be
checked for neatness and completion. Study material should be reviewed.
Homework refers to an assignment made by a teacher that will positively
reinforce a concept presented in class. Homework includes both written and
study assignments, and should be done by the student independently. While
this does not preclude parental help or interest, the assignments should not
require undue parental assistance or supervision. Ample time should be
allowed for the completion of out-of-class research assignments or projects.
The following time per night is suggested for homework; this includes both
written and study assignments:
Grades 1 and 2 30 minutes
Grades 3 and 4 60 minutes
Grades 5 and 6 90 minutes
Grades 7 and 8 120 minutes
GRADING AND TESTING
Tests, classwork, homework, class participation and independent projects
(wherever appropriate) determine report card averages. This average
should show the students and their parents what steps are necessary for
further progress. If a student is doing unsatisfactory work, parents will be
notified by the teacher before report cards are issued. If parents are not
receiving test papers or other academic communications during the marking
period, it is the responsibility of the parent to contact the teacher.
Report cards are issued on a trimester basis.. Students in grades 1-3 are
assigned letter grades for their academic subjects. Students in grades 4-8
are issued number grades for their academic subjects. Grades below 70 are
considered failing grades.
Students failing any subject for the last trimester or for the school year
will be required to attend summer school.
HONOR ROLL POLICY
In June, honors will be given to students in grade 8 who meet the following
requirements:
FIRST HONORS – Grade average of 92 or above for each subject.
A conduct and effort grade deemed satisfactory by the teacher and the
principal.
SECOND HONORS – Grade average of 85 or above for each subject. A
conduct and effort grade deemed satisfactory by the teacher and the
principal.
TEACHER-PARENT CONSULTATIONS
Teachers are not permitted to leave the classroom to answer the phone, or to
confer with parents while classes are in session. No one is permitted to go
to the classroom without the permission of the principal. If it is necessary
to bring articles of clothing, lunches, etc., leave these in the secretary’s
office.
If a parent wishes to consult with any faculty member or the principal, the
parent should call the school office or send a note to school listing days
and
times available for a conference. Requests for an appointment will be
honored as soon as possible. We recommend this communication policy for
the school:
- The teacher is the first contact.
- The principal is the second contact.
FIELD TRIPS
Field trips are not a right but a privilege. Field trips will be scheduled at
the discretion of and in the professional judgment of each individual
classroom teacher. Students who disregard school rules and regulations
will not be invited to attend this school function. The Principal has the
authority to limit participation in field trips by a parent or student.
Proper dress and good conduct are essential on all trips. Any disciplinary
infraction occurring during a field trip will disqualify the student involved
from attending any subsequent field trips.
Since a field trip is primarily a learning experience, the time spent is
considered part of the normal school day. Children who do not participate
in the field trip must attend school or be marked absent.
CARE OF SCHOOL PROPERTY
Students are asked to take care of and respect all property (this includes
parish property). Students should be trained by their parents to take care of
equipment and furnishings that are provided by the school for their use.
Failure to do so would warrant payment or replacement of equipment.
CARE OF BOOKS
It is important that students take proper care of their books. Most
textbooks,
with the exclusion of religious texts and other textbooks purchased by the
parish are provided by the Government through Act 195-Act 90. Books are
to be carried to and from school in a strong, sturdy school bag. Books must
be covered throughout the school year. (Do not place contact paper directly
on a hardback textbook). Defacing books, loss of books, and improper
handling of uncovered books will result in the student paying for the book.
The same principle will apply to library books.
HEALTH SERVICES
Students must have all immunizations required by state law prior to
registration for school. The school nurse is in the nurse’s office on a part-
time basis on the day assigned by the School District of Philadelphia.
If a student becomes ill in school, the student reports to the nurse, if the
nurse is not in, the student is to report to the school office. From there
school personnel will decide what should be done. Students who are ill may
not leave the building without an authorized person signing them out in the
office.
Emergency cards are kept on file for each student . It is vitally important
in
time of emergency or illness to have information that will enable us to
contact parents immediately and directly. These cards should include a
relative or neighbor who can be contacted in case parents cannot be
reached. We ask you to keep up-to-date any changes that are
made affecting address, telephone numbers, health, family status or a family
doctor. Please send these changes to the office immediately.
*Please Note: Under no circumstances may a student carry any
medication on his/her person or in any container or area in and around
the school property.
If a child is required to take medication for an illness, that medication
should be given at home either before or after school. Only in times of
absolute necessity may a prescription medication be given in school.
If a student requires any prescription medication during school hours, a
note from the physician or parent along with the medication in the original
bottle should be brought to the school office. School personnel designated
by the principal will distribute any medications.

The optional summer uniform may be worn from the first day of school until
November 1 and from April 15 th until the last day of school. Boys in grade
K to 8 and girls in grades 5-8 have the option of wearing the school golf
shirt with the winter uniform or wearing the full summer uniform as
described below:

must be kept neat and trimmed above the collar.
JEWELRY
Students may wear one thin neck chain with a religious medal, one watch
and one ring per hand. Small post earrings may be worn by girls only.
Only one earring is allowed per ear and the earring may only be worn on
the ear lobe. No other jewelry may be worn.
Any personal appearance that constitutes a distraction is not permitted.

SCHOOL BUS RULES
1. BE AT THE BUS STOP ON TIME. You should not have to run to
make the bus
2. WAIT FOR THE BUS IN AN ORDERLY FASHION IN YOUR
ASSIGNED AREA. Pushing, shoving, or creating a disturbance might
result in being late for school.
3. TAKE YOUR SEAT IMMEDIATELY. Remain seated while the bus is
moving or when instructed to do so by the bus driver or attendant.
4. ENTER AND LEAVE THE BUS ONLY AT THE FRONT DOOR AND
ONLY AFTER THE BUS HAS COME TO A FULL STOP. (In case of
an emergency, the driver or the attendant may give you other
instructions).
5. KEEP YOUR HEAD AND ARMS INSIDE THE BUS WINDOWS AT
ALL TIMES. Throwing objects, no matter how small, on the bus or
out of the windows is extremely dangerous.
6. SAVE YOUR LUNCH AND SNACKS UNTIL YOU ARRIVE AT YOUR
DESTINATION. Eating and drinking are not allowed on school
buses.
7. AVOID SHOUTING AND OTHER LOUD NOISES. In addition to
disturbing others, loud noises could distract the driver and cause
unsafe operation of the bus.
8. LEAVE THE BUS IN A QUIET AND ORDERLY MANNER. Join your
schoolmates in the schoolyard or go quietly to class if school has
begun.
DISCIPLINE
Holy Spirit School’s philosophy of discipline is to promote an atmosphere of
respect for oneself, others and the environment. The goal of the discipline
policy is to assist the student in making responsible choices in order to
maintain a positive teaching and learning environment. Expectations are
defined at the beginning of the school year with the cooperation and
participation of the students. Classroom rules and regulations are set up
according to the age and maturity level of the students.
Running is not permitted at any time within the school building or in moving
from one place to another on the school premises.
If class or school rules are not followed, necessary action will be taken.
These actions may include a conference with the student, conduct referral,
loss of privileges or conference with the parent.
SUSPENSION
The following offenses can result in a school suspension:
1. Repeated classroom disciplinary violations
2. Repeated violation of the school dress code
3. Insolence, disrespect or insubordination
4. Cheating/Stealing/Plagiarism
5. Vandalizing school or church property
6. Use of foul language and abusive gestures
7. Leaving school property without permission
8. Truancy
9. Fighting
10. Immorality
11. Smoking
12. Harassment or intimidation
13. Written or verbal threats
14. Bomb scares or triggering other false alarms
15. Action which endangers the well-being of student or staff
16. Offenses deemed serious by the principal
The Principal will notify parents if serious disciplinary action is
necessary.
If after receiving two out-of-school suspensions, another serious incident
should occur, the student will be dismissed from the school.
The following action will result in IMMEDIATE expulsion from school:
1. Alcohol use or possession
2. Drug use or possession
3. Possession or use of a weapon
4. Any deliberate action which endangers the well-being of a student or
staff member
5. Any action deemed extremely serious by the Principal or Pastor.
These categories do not cover every possible situation. The school is
responsible for determining what is appropriate or inappropriate behavior,
both in and out of the classroom, in the school community generally, and
outside the school community where such improper behavior affects the
school community.
Conduct by students or parents/guardians, or anyone acting on their behalf,
incompatible with the educational and religious mission of the school is
grounds for disciplinary action, including but not limited to immediate
dismissal of the student, as well as reporting the incident to the legal
authorities where appropriate.
In addition, in the case of threats of violence or harassment in any form,
including oral, written, or electronic, by a student against any member of
the
school community, the student, if suspended but not dismissed, may be
required to have psychological or psychiatric clearance before returning to
school.
Involvement of Parents/ Guardians
Parents and guardians must weigh seriously their obligation to educate
their children in an atmosphere of love and respect for God and others. The
home is the first school of the social virtues essential to any well-ordered
society
Active cooperation of parents and guardians is expected and required as
follows:
1) Sending their child to school physically fit, clean and properly
fed.
2) Assisting their child’s spiritual, academic and moral
development through careful attention to his/her report card,
Supervision of home study and behavior and cooperation with the
School in matters of activities, recreation, academic and
discipline.
3) Discovering their child’s special interests and talents so that
they may be developed to the fullest.
4) Sending a written explanation each time their child is absent
from school.
5) Setting a good example by personally refraining from any
activity that would violate Holy Spirit School policy,Catholic
teachings, or the law.
6) Taking an active role in the Home and School Association.
The failure of a parent/guardian to take seriously his/her
responsibilities in this area is grounds for action by Holy Spirit School,
including dismissal of the student.
Acceptable Use Policy for Technology Catholic Schools
of the Archdiocese of Philadelphia
Purpose
Technology is a valuable educational vehicle. Our schools are committed to
teach its faculty, staff, students and school community to work and to learn
effectively with technology and to ensure responsible use of technology. The
policy outlined below applies to all technology use including Internet use.
The Acceptable Use Policy for Technology applies to all students, faculty,
staff, volunteers or community members allowed access to school technology
resources.
The school’s goal is to prepare its members for life in an electronic, global
community. To this end, the school will:
* provide a variety of technology based tools
* teach technology skills
* integrate technology with curriculum
* encourage critical thinking and problem solving skills
* facilitate evaluation and synthesis of information
* encourage ethical practices
Responsibilities of User
Our schools will make every effort to provide a safe environment for
learning with technology including Internet safeguards. The faculty, staff,
students and school community are granted the privilege of using the
computer hardware and software, peripherals and electronic
communication tools including the Internet. With this privilege comes the
responsibility to use the equipment correctly, respect the name and
intellectual property of others and follow the policies outlined below.
Technology Use Guidelines
• Educational Purpose/Appropriate Use: All technology use and
Internet access at schools for all faculty, staff and students is provided
solely for educational purposes. Educational sites and teacher-created
assignments are to be used to enhance student learning.
Students must not access entertainment sites, for example, social
networking sites or gaming sites, except for educational purposes
under teacher supervision. Expressed permission to use the Internet
and hardware/software in any area of the school must always be
obtained.
Copyright/Intellectual Property: All sources obtained for teacher
and student work should be properly cited. Users are to respect the
rights of and the intellectual property of others in accordance with
Federal Copyright Law. Transferring copyrighted materials to or
from a school without expressed permission of the owner is a
violation of Federal Law.
• Unacceptable Use:
Users must not use equipment to harass, threaten, deceive, intimidate,
offend, embarrass or annoy any individual.
Users must not post, publish or display any defamatory, inaccurate,
violent, abusive, profane or sexually oriented material. Do not use
obscene, profane, lewd, vulgar, rude or threatening language. Do not
knowingly or recklessly post false information about any persons,
students, staff or any other organization.
Users must not use a photograph, image or likeness of any student,
employee or parishioner without express permission of that individual
and of the principal. Users must not use school equipment to create any
site, post any photo, image or video of another except with express
permission of that individual and the principal. Maintaining or posting
material to a Web site or blog that threatens a likelihood of substantial
disruption in school, including harming or interfering with the rights of
other students to participate fully in school or extracurricular activities
is
a violation of the Acceptable Use Policy and subject to the disciplinary
measure found herein.
Users must not attempt to circumvent system security, guess passwords
or in any way gain access to secured resources, another person’s files or
another person’s password.
Users must not install, move, delete, download, upload, reconfigure or
modify any software or files on school equipment without permission.
Users must not move, repair, reconfigure, modify or attach external
devices to the systems without permission.
Users must not deliberately visit a site known for unacceptable material
or any material that is not in support of educational objectives. Students
must not access entertainment sites, for example social networking sites
such as myspace.com or facebook.com or gaming sites, except for
educational purposes under teacher supervision.
Users are not to plagiarize content and may not present the work of
another as their own without properly citing that work.
Users must not violate license agreements, copy disks, CD-ROMs or
other protected media. Users must not use technology for any illegal
activity. Use of the Internet for commercial gains or profits is not
allowed from an educational site.
• Reporting: Users must report immediately any damage or change to
the school’s hardware/software that is noticed by the user.
• Electronic Devices: Users must adhere to local school policy
regarding the use of additional electronic devices including, but not
limited to, personal digital assistants (PDA), calculators, gaming
devices, cellular phones and pagers. Access will be determined by the
administrator of the school. The school’s technology policy regarding
authorization, use, responsibility, integrity, intellectual property and
monitoring will be applied to these devices.
• Administrative Rights: The Office of Catholic Education or the
school administration at any time may add additional rules and
restrictions. The school has the right to monitor both student and
employee use of school computers.
Violation of the above rules will be dealt with by the administration of
the school. Violation of these rules may result in any or all of the
following:
Loss of use of the school network, computers and software, including
Internet access. The student will be expected to complete work on a
non-networked, stand-alone computer system.
• Issuance of demerits/detentions, if applicable
• Disciplinary action including, but not limited to, dismissal and/or
legal action by the school, civil authorities or other involved parties.
Legal Custodies Issues
Parents are asked to inform school personnel when legal custody of the
child(ren) resides with one parent. It is important for the school to have a
copy of the custody decree. This will help school personnel to make effective
decisions when the need arises.
Those individuals who have legal custody of the student may attend school
meetings, participate in educational decisions and review educational
records regarding that student. Persons who do not have legal custody
(including those with visitation rights but not legal custody) have no such
educational rights and may not participate in these matters.
The School requires parents/guardians to sign an agreement, regarding the
administration of parental participation issues and payment of tuition, as
follows:
We are the parents/guardians who have legal custody over
_______________. No other person has such a right. Only we are legally
entitled to make decisions concerning the education of
________________. We agree that no other person is entitled to
participate in the issues surrounding___________________’s attendance
at school, including participation in school meetings. We agree that no
other person is permitted to attend a school meeting, and the school will
not conduct a meeting unless only both or one of us (if we so agree
beforehand) is present. We understand that separate meetings for each of
us will not be held.
Recognizing that requiring the school to provide duplicate copies of school
grades, notices and other related educational materials and notices is
administratively burdensome, we agree that the school is required to send
only one set of such materials to the parent/guardian identified below, and
that we retain the responsibility of communicating such information to
each other. We understand that the school is not required to send such
material to any other person.
We also notify the school that, as indicated below on the dates listed,
___________________, may be released to me (us) at dismissal.
________________________ may be picked up before dismissal when
necessary to do so as identified below on the days listed. In an emergency,
or when we cannot be reached, _____________________________may be
released to those persons whom we have identified on the emergency
contact card. We agree and represent that these dates below reflect when
we have physical custody of _____________________ as reflected in the
custody order and/or custody agreement we have provided to the school.
We agree that we are jointly and independently responsible to pay tuition,
and that our failure to pay tuition constitutes grounds for dismissal of our
student.
