under the guidance and supervision of the staff and those designated by the
staff as supervisors.

Field Trip Permission Form/7th Grade – “Disney on Ice”

I/we, request
and give permission for my child to go on this Field Trip. This permission
includes all related programs or events associated with the field trip. In
consideration for my/our child’s participation, I/we and my child agree and
understand that we assume the risks inherent in the field trip, and with full
knowledge of the risks, we agree to release and hold harmless St. Francis of
Assisi School at St. Francis of Assisi Parish in Norristown, and the
Archdiocese of Philadelphia and their employees and representatives, from
claims arising or related to my/our child’s participation.
My/our child understands and
agrees to abide by all rules and regulations established by the school
pertaining to such field trip.
I/we consent and give
permission for emergency medical care for my/our child that may be needed as a
result of my/our child’s participation.

Each student must return
this signed permission form before being permitted to participate on the field
trip. If both parents have custody, they both must sign. If one parent is out
of town or otherwise unavailable, this fact should be noted on the form. If
only one parent has custody, permission must be obtained from this parent with
acknowledgment that this parent is sole custodian of the child. If there are 2
guardians, permission must be obtained as indicated above for two parents. If
there is no parent and only one guardian, permission must be obtained from the
sole guardian with the acknowledgment that he/she is the sole guardian.
