Freshman

Transition Planning

Parents and the student should:
- secure a state photo identification card
- inquire about S.S.I. eligibility
- register with MH/MR if appropriate
- determine what services are appropriate
- inquire about services in your area
- start developing independent living skills in the home
- identify necessary accommodations to be a successful learner
  and worker
- explore post-secondary options and admission requirements
- identify interests and options for future living arrangements,
  including supports
- investigate assistive technology devices necessary for school
  or work

Parents should:
- begin planning for the student's financial needs
- involve the student in extra curricular activities in the community 
  and at school
- develop money management skills
- know that ADULT SERVICES are not mandatory.  They ARE
  BASED ON ELIGIBILITY.  The STUDENT MUST REGISTER 
  before services are provided and to make sure funding is
  available after graduation from high school.

Student should:
- be able to explain disability and accommodations that are 
  necessary
- learn to use public transportation
- be able to communicate personal information in case of 
  emergency
