Sophomore

Transition Planning

Parents and student should:
- Develop the Individual Transition Plan section of the I.E.P.
  - address goals for adult life, post secondary education, and 
    work with the assistance of  school personnel and adult
    service providers
- identify transportation, attendant care, and functional needs for
  a job or vocational placement, college or training program,
  independent living, and leisure activities
- link with community services such as O.V.R. , MH/MR, Center
  for Independent Living
- look at the student's strengths and needs
- initiate community work or volunteer experiences
- gather information post secondary programs and support 
  services that they offer

Parents should:
- Remember adult services ARE NOT mandatory.  They are 
  based on eligibility.

Student should:
- match career in terests and skills with course work and
  community work experiences
- learn and practice appropriate social skills for different  settings
- begin a resume
- continue to work on independent living skills (budgeting, 
  cooking, home repair, shopping)
