Transition Planning

Parents and student should:
- establish long-term goals
- begin to visit agencies that may be appropriate for the student
- identify appropriate post secondary educational options and 
  any necessary accommodations
- arrange for testing that is required for the agency, college, or
  post secondary program that the student is planning to attend
- make sure the student has register with the appropriate 
  agencies
- identify residential supports

Parents should:
- identify transportation needs and teach transportation skills
- Remember adult services ARE NOT mandatory.  They are
  based on eligibility.

Student should:
- continue community based experiences
- practice interviewing skills
- develop a resume
- secure employment in a desired job with support if necessary
- take responsibility for arriving on time for class, work, activities,
  appointments
