Students MUST bring in the vaccination consent form that was mailed ASAP Additional copies are available in the main office and at: http://cpsed.net/bainhome/ Click on the Nurse's Page Vaccine will not be in doctor's offices until after all the school clinics are done. If a student does not have their form in, they will not get the flu shot. This is different than the seasonal flu shot. Forms will not be accepted the day of.
