WHAT?? Job Shadowing is a day designated to provide students an opportunity to shadow adults on the job and learn firsthand about the career they are interested in pursuing.
WHEN?? The student will shadow on __________________, 2009. You must have the completed application in the school guidance office no later than ____________________, 2009.
WHO?? All qualified 8th grade students from DMS are expected to participate. To qualify to job shadow, you must maintain a C average (75 or above) each 6 weeks; you cannot have any suspensions (in-school or out-of-school); no more than 8 penalty points (each discipline event such as a detention or bus write-up receives a point); have a good attendance record (Good attendance should mean no more than 4 unexcused absences and no more than 9 excused absences; however, this will be addressed on an individual basis; if there is a medical reason for multiple absences, it will be taken into consideration).
HOW?? Contact an employer and get approval to shadow him/her on ___________________, 2009, for the entire school day. Ask the employer where and what time you should meet him/her and how you should dress for the day. Give him/her a copy of the following 3 forms: “Workplace Host Responsibilities,” “Benefits to Employers” and “Workplace Host Evaluation.” Be sure to get his/her signature on the commitment form at the bottom of this application. You must return this form and the attached permission and medical authorization forms by ______________________, 2009, to ____________________ (your homeroom teacher) with all the appropriate signatures.

Your signature below indicates your permission and commitment to provide a meaningful shadowing experience for the entire school day for this student.
