The parent is responsible for delivery and pick-up of their child. |

Parents WILL support the staff at MAP with their child's discipline problems. |

Parents may use 5 notes that will cover 5 days of absences per semester for absences. After that they must have a doctor's note or funeral director's note (only for immediate family members), etc. |

If my child requires medication on a daily basis at school, it is the parent's responsibility to bring in proper documentation to have that medicine kept at school. (Doctor must fill out a form you may get from the MAP Academy Secretary) |

(For Over-The-Counter medications, there is another form the Secretary will provide upon request) |

|
Parents understand that if their child destroys school property, personnel property, or community property, they will be responsible for reimbursing such parties. |
|
|

|
Parents understand that for each tardy (the student loses 15 points for each late entry until 5th tardy) after 4 (in the mornings) their child will lose 50 points per late entry. |
|
|
7 |
|
Parents understand that if their child owes ANY points on their last day at MAP they must remain at MAP for that entire day to earn them. |
