Please remember that all Kindergartners must have an updated Tennessee Green 
Card with all Kindergarten Immunizations and a blue physical form attached 
BEFORE the first day of school!!!

Please return all dental screening forms as soon as possible. Dental Sealant 
set for October 3, 2008.

Vision, Hearing, Blood Pressure screening for all 2nd, 4th, 6th, and 8th 
graders set for October 24, 2008
