Enrollment & Registration Requirements

1. Proof of residence or approved out-of-zone waiver

•      Electric Bill
•      Water Bill
•      Natural Gas Bill

Not Acceptable:

• Telephone Bill
• Driver's License
• Credit Card Statement
• Lease agreements made more than 45 days prior to registration

2.  Immunization Record on a Tennessee Green Form

•     2nd MMR must be included
•     Physical required if entering Tennessee school for the first time 

3.  Custody

•      If applies, proof of custody is required signed by a judge or judicial referee
•      In cases of divorce, child must attend school in the zone in which primary custodian resides
