The State of Texas, Texas Board of Health and the local school board require the following
immunizations:
DPT (Diphtheria, Pertussis, Tetanus):
At least three (3) doses. Last dose received on or after 4th birthday
Polio:
At least three (3) doses of Polio vaccine (OPV or IPV) are required, provided at least one dose has
been received on or after the 4th birthday.
Rubeola:
(Measles) Two (2) doses, one being on or after the first birthday. The second dose must be at least
30 days after the first dose.
Mumps:
One (1) dose of vaccine received on or after the first birthday.
Rubella:
One (1) dose of vaccine received on or after the first birthday.
Hepatitis B:
Three (3) doses are required.
Varicella (Chickenpox):
One (1) dose of Varicella (Chickenpox) vaccine on or after the 1st birthday or documentation of
previously having the illness.
Hepatitis A:
Recommended; not required.
MMR:
Two (2) doses meets requirements for Measles, Mumps, and Rubella.
***The day, month, and year of vaccine administration must be recorded on all new school records.
THE IMMUNIZATION REQUIREMENTS MUST BE MET BEFORE A STUDENT WILL BE ALLOWED TO REGISTER
