These are the documents we need back by May 24th. They are the first 3 listed here: Physical,
Clothes, and Rules/Regs form.The only one not available here for download is the emergency card
which is available from the coaches. Parents please fill out completely and sign where
appropriate (the athlete will have to sign most of the documents, as well). Everything is due
back to us before the kids leave for the summer except page 3 of the 2013 physical form. That
page is for your doctor and may be returned at the parent meeting on August 22. Please e-mail us
