Immunization Requirements for 2012-2013

Incoming Kindergarten:
*Hepatitis A  - Two doses with the 1st dose on or after the 1st birthday

*MMR Vaccine  - Two doses with the 1st dose on or after the 1st birthday

* Varicella Vaccine  - Two doses with the 1st dose on or after the 1st birthday (validation of the disease accepted on a seperate form)

Must have verification of these shots on the 1st day of school in August, 2012.

Incoming 7th Grade:

*Booster dose of Tetanus/Diphteria/Pertussis

If at least five years has passed since the last dose of tetanus containing vaccine.

*Meningococcal Vaccine  - one dose for entry into 7th grade

*Varicella Vaccine  - two doses received on or after the 1st birthday

Must have verification of the above shots on the 1st day of school in August, 2012.
