questions regarding either. Registration form must be received by May 29,
2008 to ensure your child recieves a ball and a t-shirt!)

Assumption of Risk/Release of Liability
It is understood that medical insurance covering injuries of any nature
incurred during the 2008 Lady Lion Summer Basketball Camp is not provided.
The undersigned hereby releases the Lockhart Public Schools, its
successors, assigns, officers, agents, and growing out of or result from
the participation in the basketball camp, and that camp staff will not be
responsible of accidental injury at the camp. All participants should be
covered by their own insurance policies.
We, as parents/guardians of the above named child, hereby grant
permission of said child to participate in the Lady Lion Summer Basketball
Camp 2008, and acknowledge the fact that my child is physically able to
participate in camp activities.
