Parent/Guardian Assignment
Due September 4th
 
In a Million Words or Less....
 
Please tell me about your child. It would be great to be aware of the things that you believe are important to know about your special child.   Feel free to write as much or as little as you would like to share.  Below are some questions to get you started.  You may email it, type it or write it by hand.
 
Please use a blank sheet of paper, not this sheet.
 
What are his/her strengths and weaknesses?
 
What is his/her favorite activity?
 
What are your child’s greatest accomplishments?
 
What does your child like to do for fun?
 
What is his/her favorite subject?
 
How does your child feel about homework or projects?
 
Are there any concerns you would like for me to know?
 
What would you like your child to accomplish in 6th grade?
 
What motivates your child?  Recognition?  Competition?  Desire to learn?
 
What is the most important thing that I should know about your child?
