Welcome to Lakeland Elementary School's Department of Coordinated School Health! The Lakeland Elementary Coordinated School Health program covers eight areas of Health and Wellness, found below. We strive to improve the overall wellness of our school to ensure that all students achieve their maximum potential.

    Parents of incoming 7th graders--Remember that the state requires your child to have their TDAP (tetanus-diphtheria-and pertussis) booster shot and their second varicella (chickenpox) shot to enter 7th grade!  The Meningitis vaccine is recommended, not required for 7th grade.
    You MUST bring your child's updated immunization form
    on the TN state certificate to registration on August 1st.

    *For entry to 7th grade, your child needs a TDAP (tetanus-diphtheria-and pertussis) booster and the second varicella (chickenpox) vaccine.
    -If your child has a credible history of the chickenpox disease, they may not need the vaccine. Instead of writing the date of the vaccine on the certificate, the physician will write the date that your child had the disease.
    *In the bottom box of the certificate, labeled Section 3, the physician will mark the box that says "E) Complete 7th Grade or Higher."
    *The Shelby County Health Department has an immunization clinic beginning 7/24/17. They can give your child any vaccines they are missing and/or they can transfer your records to the required TN Immunization Certificate. They are located at 6170 Macon Rd, 38134 or.     *Doctor's offices are busy during the back-to-school rush. If your child needs shots or you need them to print a copy of your child's shot record, it can take up to a week.
    Medical- Physicians (MD or DO) or Public Health Nurses are authorized to indicate specific vaccines medically exempted on the certificate. Other vaccines remain required.

    Religious- This exemption requires a signed statement by the parent/guardian that vaccination conflicts with their religious tenets or practices. If the child needs documentation of a health examination for the school, it must be noted by the healthcare provider on the immunization certificate. In that case, the provider should check the box in section 1a. that the parent has sought a religious exemption to explain why immunization information is absent or incomplete. (Per the TN Department of Health website)
    -Please note, the State of TN does not recognize personal beliefs as a reason for exemption. This means that you can not claim a personal, moral or philosophical belief against vaccinations to obtain an exemption.
