PHYSICAL EDUCATION DISCLOSURE

I. COURSE DESCRIPTION

The goal in Physical Education is to help all students develop positive 
attitudes regarding physical education and the importance of healthy 
lifestyles.  Each student will be required to participate in a variety of 
lifetime skills and activities and to be a team member in a variety of team 
sports.  Students will learn the importance of personal fitness, 
sportsmanship, leadership, and social skills.

II.  GRADING

Grades in Physical Education are determined by the student's attitude, 
attendance, dressing in the required Physical Education uniform, 
participating in daily activities and required physical education tests, 
giving one's best effort at all times, and following all 
classroom/school/district instructions, 
rules and policies. 

100-90 = A 
89-80  = B
79-70  = C
69-60  = D
59-0   = F

Students recieve 10pts daily based on the following

1. Required P.E Uniform and class participation   10pts
2. Uniform infraction                             8pts
3. Non-Dress                                      6pts
4. ABSENT                                         0PTS             	
                              		
CITIZENSHIP GRADE

Satisfactory citizenship towards administrators, teachers, custodians, and 
peers is important.  This includes appropriate behavior, appropriate 
language, 
punctuality, attendance, and adherence to all classroom/school/district 
rules 
and policies.


ACADEMIC AND/OR CITIZENSHIP FAILURES

Make up for Physical Education absence is avalible upon request.

 
III. PHYSICAL EDUCATION UNIFORM

1.  Plain white T-shirt.
2.  Plain black athletic shorts.(No Logos).
3.  White athletic socks.
5.  Athletic Gym shoes.  Non-marking soles.


IV. MEDICAL PROBLEMS

Students who have special medical problems or physical limitations need to 
bring a note/letter written by their physician.  The note/letter must 
include the name and phone number of the physician as well as the nature and 
duration of the illness or limitations so substitute activities can be 
assigned.


V.   ACCIDENTS OR INJURIES

All accident and injuries must be reported to the coah immediately. 

VI. REFERRALS TO SCHOOL ADMINISTRATION

Students will be taken directly to the school administration for defiance, 
fighting,
stealing, vandalism, pantsing, or sexual harassment.


VII. GENERAL RULES

1.  Be on time to Physical Education (in the gym).
2.  Be in proper gym clothes and in roll call five minutes after the tardy 
    bell.
3.  Respect others and their property.
4.  Follow directions the first time.
5.  Read and obey all posted signs in the gymnasium and the locker rooms.
6.  Do not kick, vandalize, or tag lockers.
7.  Do not write on or vandalize bulletin boards or bleachers.
8.  Do not hang on the basketball nets or rims.
9.  Do not pull down or hang on the volleyball nets.
10. Leave watches and jewelry in your physical education locker.
11. Do not bring food,drinks or gum into the gymnasium.
12. After dressing in your school clothes, do not leave the gymnasium 
    until the bell rings.

HEALTH EDUCATION DISCLOSURE 


I.  COURSE DESCRIPTION

This one semester course assists students in the continual development of 
knowledge and skills needed to improve the quality of their lives.  Health 
Education must do more than merely provide information.  In order for it to 
be worthwhile, the information must be presented in such a way that it 
influences students to take positive actions about their own health.  This 
course offers students an opportunity to learn decision-making skills as 
they relate to health practices, products and services.  Emphasis is placed 
on the physical, social and mental dimensions of individuals, thus helping 
students become better informed consumers of health-related information.

·Drugs, Alcohol and Tobacco

III.  GRADING PROCEDURES

As grades are generally of the most concern to the parents and students, the 
following scale will determine grading policy.

95% -100    = A		73% - 75% = C
90% - 94% = A-		70% - 72% = C-
86% - 89% = B+		66% - 69% = D+
83% - 85% = B		63% - 65% = D
80% - 82% = B-		60% - 62% = D-
76% - 79% = C+	        Below 60% = F  

IV.  CITIZENSHIP GRADES

This grade is given based on classroom conduct.  It will be based on the 
number of TARDIES and ABSENCES, as well as BEHAVIOR in the classroom.  As 
class conduct ultimately affects academics, it is expected that students 
become more aware and responsible for their own behavior.


V.  MAKE UP WORK  

·Assignments will be posted on my web site daily.  I expect these to 
 be recorded in the student’s daily planner.
·Assignments turned in late will recieve half credit.
·Assignments missed due to absence or activity must be turned in the
 following class.  


VI.  Required Material  

·Pencil or Pen – Dark ink only (blue or black)  NO NEON COLORS.
·Assigned textbook (students are responsible for loss or damage).
·Accordion binder including paper (can be used for other classes).
·Optional – colored pencils, highlighter (these are used on various 
 handouts).

VII.  CLASSROOM RULES

·Be seated when the bell rings; stay seated all period.
·Do not create problems for yourself or others.  
·If you create a problem you are responsible for solving it.
·Cheating or plagiarism will result in the loss of grade for that 
 particular assignment.


VIII.  CLASSROOM BEHAVIOR/CONSEQUENCES
The academic climate in any classroom is an integral part of instruction.  
Those students who alter that climate by disrupting other students will be 
warned once.  The second time, the parents will be notified.  On the third 
infraction the student will be sent to the Vice Principal and a conference 
will be called with the student and parent(s) and teacher.
