	                        DISCLOSURE STATEMENT
	                        MS. MALEE JERGENSEN
                              CONSULTAION: 2nd Period
	                    
STUDENTS AND PARENTS:

Please be advised of the following daily assignments and class rules for your 
students math class at Wasatch Jr. High School.

	1. Bring loose leaf binder, paper, composition notebook.         
           MECHANICAL PENCILS, planner, assigned work everyday,
           and colored pen everyday.  Citizenship is lowered daily when
           not prepared.
       	2. Follow directions the FIRST time they are given.
	3. Be on time ready to start when the bell rings with pencils         
           sharpened. 3T= "2" in citizenship, 4T= "1", 5T= "0" which ='s 
           (No credit). After your 4th tardy you will receive an 8th period   
           for every tardy following.
        4. Keep hands, feet and objects to yourself, don't invade        
           other peoples space.  BE RESPONSIBLE FOR YOUR BEHAVIOR    
        5. Raise your hand and wait to be called on before speaking.  Be      
           courteous to others when they are speaking, be HONEST in all your 
           dealings.
	6. No CHEATING in the class or outside the classroom,                 
           citizenship will be lowered to a "0"
	7. PLEASE bring a note to excuse your absence the day you             
           come back to school. Turn your note into the counseling center     
           before school begins. Please be responsible.
        8. NO CELL PHONES, MPIII PLAYERS, are allowed out in my room.  Phones 
           are available in the front office for your use.  They will be taken
           away from your student and given to the office.
        9. When a SUBSTITUTE is in my room they are in charge.  If you get in
           trouble with the sub you will receive an ASD and your citizenship
           will be lowered.


If a student breaks a rule, the following consequence will occur:

     1.  FIRST TIME--1st name written down
     2.  SECOND TIME--last name written down, citizenship is lowered
     3.  THIRD TIME--Jergensen's first name written down
     4.  FOURTH TIME--Jergensen's last name written down, citizenship is
         lowered.                   		
     5.  FIFTH TIME--1 HOUR DETENTION, PARENTS CONTACTED, "0" in citizenship. 
     .           

Included in my classroom discipline plan are ways to positively reinforce 
students who behave appropriately.  I will reward them with the following:

     1.  A "4" in citizenship.
     2.  Jerg's good time coupons
     3.  Extra credit (assignments, quizzes, and tests).
     4.  Extra HALL PASS                                           
     5.  Listen to the radio, or play C.D.'s (all are my choices)
     6.  Positive notes and phone calls home to parents.

     
To help you understand the grading procedure I will be using, please note the 
following:
	1. If your students score is a zero "0" for an assignment, it         
           means that he/she was in class the day the assignment was          
           made, and the day it was due and he/she either failed to           
           have the assignment or they had no problems right.
        2. When a student returns to class after an absence, the              
           assignment that was made in class the last day of his/her          
           attendance is DUE UPON RETURN.  If he/she is not prepared          
           with the assignment at that time, they will also receive           
           a zero.
        3. IMPORTANT NOTE:  When a student returns from an absence,           
           he/she should copy the assignment into their agenda, the           
           assignments are listed on the web page.

           If you don't have access to a computer at your home you can 
           call your study-buddy.  Please don't call the school.
        4. VERY IMPORTANT:  When a student is absent from my class because 
           of a school function they need to check the web page and get the 
           assignment.  They are NOT excused from the work.

GRADING:  Tests:  30%, Quizzes: 15%: Journal writing, vocabulary, and note
          taking: 10%: Daily assignments and four notebooks: 45% (Notebooks 
          are turned in at the end of each term.  They contain all assignments
          tests, and quizzes.  NOTHING IS THROWN AWAY!!  Most of the quizzes
          will be taken from assignments, notes taken in class, and journal
          writing.  Please come prepared daily to use your notebook and agenda
          with all your assignemnts and notes.

$$$$$$ THINGS YOU WILL NEED TO PURCHASE:   Loose leaf binder, lined paper,
          MECHANICAL PENCILS, lead, or package of pencils, pink eraser,red
          pen, dividers to go in the binder, colored pencils, and a TI-34 
          MultiView calculator. Student Planner (school will provide)   
             
       
          In order for this plan to have its greatest effect. I need your
