HEALTH TREATMENT PLANS
If your child has a medical condition, please make sure that you have
submitted a Health Treatment Plan for that condition (Asthma, Allergy,
Diabetes, Emergency.) Forms are located on the Forms and Regulations page
of this site or on the PWCS website. Please make sure that your child's
physician has signed the form. Once the plan is complete, I'd like to meet
with you to discuss your child's needs. You may also drop off your child's
medication at that time.
ILLNESSES
Please inform your school nurse if your child has an infectious disease,
recent hospitalization for illness or an orthopedic injury requiring a wheel
chair so that we may make the proper accommodations upon the student's
return to school.
MEDICATIONS AT SCHOOL
If you need to send medication to school, it must be kept in the school
clinic and a Medication Authorization Form should be on file. This includes
over the counter medications such as Tylenol, Ibuprophen, and cough drops.
Students may not carry any medications on their person at school. Please
use the links on the Forms and Regulations page on this site to access the
medication form. The form must be filled out completely and brought to
school with the medication. Parents must deliver the medication to school.
IMMUNIZATIONS
If your child is in the process of completing a series of immunizations
(such as Hep B) - please be sure to return to your physician when the next
booster is due. You may drop off, mail, or send the updated immunization
forms to school with your child so that we can update your child's records.
