Below are listed all the forms you will need to register your student for
sports. Please print one of each of the following:
Physical Form (good for 2 years)
Athletic Registration,
Concussion Compliance,
Emergency Med Treatment,
Code of Conduct Agreement and
Caution Statment form/s for all the sports your student wants to register
for.
Don't forget the parent flyer!
Your student will also need to purchase an ASB card (Associated Student
Body)
This
card gives your student access into sports for the year, $1.00 discount on
dance
tickets and $2.00 off the full price of the yearbook! Membership may be
purchased
in the Main Office with Mrs. Harriott (check or cash) or online with a
credit/debit card. Copy and past the link below to your search engine to go
to
the
online payment option (Intouch Receipting System)for Clover Park School
District.
You will need your students 6 diget student id number (on their lunch card
or ID
card)
