LETTER OF RECOMMENDATION PRELIMINARY REQUEST
NAME_____________________________________________

In order to write a meaningful letter of recommendation, counselors and
teachers must know as much about you as possible. It is to your benefit to
spend some time answering these questions. This is not bragging, but about
letting people and colleges know who you are and what you are good at.
Include any information that you would like to see included in the letter.
If I don’t know what makes you special, I can’t tell anyone else. Specific
examples help to demonstrate what you say.

The accomplishments that I am most proud of are:
1_______________________________________________________
 2. ______________________________________________________
 3. ______________________________________________________

Three words that best describe me are:
1. ___________________________________________
2. ___________________________________________
3. ___________________________________________

Answer the following questions in short statements. The more someone knows
about, the more individualized the letter of recommendation can be.

1. Leadership/Group contributions: Describe examples of your leadership
experience in which you have significantly influenced others, helped resolve
disputes, or contributed to group efforts over time. Consider
responsibilities to initiatives taken in or out of school.

2. Knowledge in a field/creativity: Describe any of your special interests
and how you have developed knowledge in these areas. Give examples of your
creativity: the ability to see alternatives; take diverse perspectives; come
up with many, varied, or original ideas; or willingness to try new things.

3. Dealing with adversity: Describe the most significant challenge you have
faced and the steps you have taken to address this challenge. Include
whether you turned to anyone in facing that challenge, the role that person
played, and what you learned about yourself.

4. Community service: Explain what you have done to make your community a
better place to live.
Give examples of specific projects in which you have been involved over time.

5. Goals/task commitment: Articulate the goals you have established for
yourself and your efforts to accomplish these. Give at least one specific
example that demonstrates your work ethic/diligence.

6. What points or qualities do you feel I should stress in a letter of
recommendation?

7. Are there any “extenuating circumstances” that I should include in the
letter?

8. What additional information should I know to write your letter of
recommendation?

(Some of these questions are taken from the Oregon State University
admission application)
