Dear Parent/Guardian,
Horace W. Porter School offers several opportunities for your child
to participate in extracurricular athletics. If your student will
be entering 6th grade next year, you should look over the expectations
now in order to provide plenty of time to make appointments prior to the Fall
sports season. Beyond the age and skill requirements for participation in
organized sports at Porter School, physical and academic requirements must be
fulfilled. The physical and academic requirements for sports eligibility are
stated in the student handbook, however the following information should help
to make expectations more clear.
ELIGIBILITY FOR EXTRA-CURRICULAR ACTIVITIES
Students may not have a failing grade in any graded subject. Grades below 65
on a report card will result in suspended eligibility from participation in
after school activities.
Students whose grades fall below a 65 average at the time of progress reports
are automatically required to complete a probation contract.
Teachers may recommend that a probation contract be used for students whose
grades fall below a 65 average in any graded course at any time between
progress reports and quarter grades.
Eligibility will be reinstated if the following progress reports and/or
quarter grades meet the 65 standard in all graded courses.
If you have a question about the current status of your childs
eligibility you may want to explore the following:
1. Has your child had a physical examination by your family physician within
two years of participation in the desired interscholastic activity? Does
Porters Nurse Coordinator have a copy of the examination form?
a. If they have not had a physical exam within two years, make an appointment
for them prior to any practice or tryouts.
b. If they have had a physical exam within two years make sure to have your
Physicians office fax a copy to the Nurse Coordinator for their records
(School Fax # 228-8592).
2. Have you and your child completed a health history questionnaire for each
interscholastic season (i.e. one for soccer, another for basketball
)?
Please notice that the parent and student must sign the form.
3. Is your child currently passing all of their graded classes?
Student athletes must be passing all graded courses with a 65 or higher in
order to maintain eligibility. Students who do not maintain passing grades
will lose eligibility at the end of the marking period. Their eligibility
will be reinstated if the following progress reports and/or quarter marks
meet the 65 standard.
4. Where can I get copies of the Health History Questionnaire and Physical
examination form?
Students receive copies at sign up sessions during their lunch waves prior to
each season and can also pick them up at the Nurses office. You can also
download copies on the school website at hwporter.org by clicking on student
activities and sports eligibility. Doctors offices also keep state
approved physical examination forms on hand.
III. Transportation to and from sports events
*Teams will be transported by bus to and from all off-site competitions.
*Students who are being picked up at away competitions and will not be
returning on the bus with the rest of the team will be required to report to
their coach with their parent/ guardian prior to departing any event.
*If a student is getting a ride from a parent other than their own, they must
submit written permission to the main office at the school prior to the
teams departure from Horace W. Porter.
*If arrangements are made between parents to carpool and pick-up each-others
children throughout a season, a written note with the names of the
appropriate parents should also be submitted to the office, stating that this
will be an ongoing arrangement.
