| We welcome new students to Sarah Smith Elementary throughout the school
year. In order to enroll at Sarah Smith, the following registration
requirements are needed:
1. Immunization Form (3231) signed by the health department official or a
licensed Georgia physician.
2. A certificate of ear, eye, and dental screening (Georgia Department of
Human Resources Form 3300) signed by the health department official or a
licensed Georgia physician.
3. A government-issued birth certificate. According to Georgia law,
children must be 5 years old on or before September 1 in order to enroll
in kindergarten. The birth certificate should be the original with the
state embossed seal. A copy will be made for the student's file and the
original returned.
4. The original Social Security card of the child. A copy will be made for
the student's file and the original returned.
5. Academic records, if applicable. Examples would includepast report
cards, the withdrawal form from the previous school, standardized test
scores, any previous placement testing results, etc.
6. Proof of residency. To enroll at Sarah Smith Elementary, the student's
parent or legal guardian must be a resident in the attendance zone of the
school. Appropriate records for verification must include a Georgia
Power bill (the original), an affidavit of residency (obtained at the
school office), and one other document from the following list:
- Home mortgage payment book
- Apartment lease showing name of parent/legal guardian
- Homeowner's insurance
- Current paycheck stub with address shown
- Driver's license showing parent/legal guardian's current address
- Copy of sales contract (if house is under construction or being
purchased)
- Receipt to have utilities connected
