Kindergarten Registration
For Parishioners: Tuesday February 26th, 2008 School Hall
For Non-Parishioners & Non Catholics: Wednesday, February 27th, 2008 School Hall
Required Documents: Birth Certicate, Baptismal Certicate & Record Book of Shots
Eligible Age: Must be 5 years old by September 1st, 2008. NO EXCEPTIONS!
Health Immunization Records (Baby Shots) - Subject to change in which you will be notified:
Diphtheria & Tetanus -(DPT) 4 shots each. One shot of each must have been given AFTER the 4th birthday.
Polio: - 3 oral doses OR 4 shots. (Please make sure your doctor indicates which type of vaccine your child received - OPV or IPV).
Hepatitia B - 3 shots.
Rubella (Measles)* - 2 shots. Both must have been given AFTER the 1st birthday.
Mumps & Rubella (German Measles)* - 1 shot of each must have been given AFTER the 1st birthday.
Varicella (chicken pox) - or history of disease. Need physician verification.
*Measles, Mumps, & Rubella - are often in a combination shot called MMR - 2 doses of MMR after the first birthday meet the requirements for these vaccines. Records must include dates: MONTHS,DAY & YEAR.
Please pass this information on to your relatives and friends who have children of Kindergarten age.
If you know of children who plan to register at St. Nicholas School in grades other than Kindergarten, they must apply by February 27th, 2008. Space available in all grades, child's behavior and academic grades will determine acceptance in St. nicholas School. PLEASE BRING A COPY OF THEIR LAST REPORT CARD.
IF FAMILY IS NOT REGISTERED AT ST. NICHOLAS CHURCH, A LETTER OF PERMISSION TO ENTER ST. NICHOLAS IS REQUIRED FROM FAMILY'S PASTOR.
